JUNIOR HIGH SCHOOL ADMISSION REQUIREMENTS

For Grade 7 Applicants
1. Duly accomplished Application Form with recent 2”x2” picture (studio taken with white background and name tag)
2. Grade 6 Certificate of Enrollment (affixed with school seal)/Enrollment Registration Form (affixed with school seal)
3. Admission Fee: Php 600.00

For Transfer Applicants
1. Duly accomplished Application Form with recent 2”x2” picture (studio taken with white background and name tag)
2. Certified True Copy of Form 138 (with Final Rating) of previously completed Grade Level for evaluation of the JHS Principal
3. Admission Fee: Php 650.00
4. Certificate of Good Moral Character

ADMISSION PROCEDURE

For Grade 7 Applicants
Step 1: Accomplish the Admission Form and supply the necessary information and attach the requirements;
Step 2: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 3: Submit the accomplished Application Form with complete attachments at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

For Transfer Applicants
Step 1: Proceed to the Junior High School Principal’s Office, St. Albert the Great Bldg. for the evaluation of Form 138.
Step 2: Accomplish the Application Form and attach the requirements.
Step 3: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 4: Submit the accomplished Application Form with complete attachments at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
