Junior High School Admission Schedule for S.Y. 2023-2024 

Requirements for Grade 7 Junior High School Admission Test

1. Duly accomplished Application Form with recent 2” x 2” picture (studio taken with white background and name tag).
2. Grade 6 Certificate of Enrolment (affixed with school seal)
3. Admission Fee: Php 550.00
4. For Transfer Applicants (Grade 8-10): Certified True Copy of Form 138 (with Final Rating) of previously completed Grade Level for evaluation.

Procedure for New Students:

The Applicant shall:

Step 1: accomplish the Admission Form by supplying the necessary information and attach the requirements.

Step 2: pay the Admission Test Fee of Php 550.00 at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

Step 3: submit the accomplished application form and Official Receipt to the Junior High School (JHS) Guidance Counselor at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

Procedure for Transferees:

The Applicant shall:

Step 1: proceed to the Junior High School Principal’s Office, St. Albert Bldg. for evaluation of their Form 138.

Step 2: accomplish the Application Form (except the School Record) and attach the requirement.

Step 3: pay the Admission Fee of Php 550.00 at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

Step 4: submit the accomplished Application Form and Official Receipt to the Junior High School (JHS) Guidance Counselor at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
