REQUIREMENTS FOR SENIOR HIGH SCHOOL

For Grade 11 Applicants
1. Duly accomplished Application Form with recent 2”x2” picture (with white background and name tag)
2. Grade 10 Certificate of Enrolment (affixed with school seal) if applying for Grade 11;
3. Admission Fee: Php450.00

For Transfer Applicants/Applicants with USTET Results
1. Duly accomplished Application Form with recent 2”x2” picture(with white background and name tag)
2. Please submit the Certified True Copy of Form 138 (with Final Rating) of previously completed Grade level
3. Admission Fee: Php450.00

For Applicants with USTET Results: Please submit your USTET Result for Evaluation

PROCEDURE

For Grade 11 Applicants
The applicant shall:
Step 1: accomplish the Admission Form by supplying the necessary information and attach the requirements.
Step 2: pay the Admission Fee at the Cashier’s Office, St. Thomas Bldg., UST-Legazpi, Rawis Campus.
Step 3: submit the Application Form and Official Receipt to Senior High School (SHS) Guidance Counselor at the Office of Guidance and Testing (OGT), St. Thomas Bldg., UST-Legazpi, Rawis Campus.

For Transfer Applicants/Applicants with USTET Results
The applicant shall:
Step 1: proceed to the Senior High School Principal’s Office, St. Thomas Bldg. for the evaluation of their Form 138 (for transfer applicants) USTET Results (applicants with USTET Results);

Note: Transferees must leave their endorsement slip for the Principal’s Office of Senior High School
Step 2: accomplish the Application Form and attach the requirement;
Step 3: pay the Admission Fee at the Cashier’s Office, St. Thomas Bldg., UST-Legazpi, Rawis Campus
Step 4: submit the accomplished application form and official receipt to the Senior High School Guidance Counselor at the Office of Guidance and Testing(OGT), St.Thomas Bldg., UST-Legazpi, Rawis Campus.
